NDIS

A and D In Home Care is a registered NDIS provider, working with all NDIS participants, including Self-Managed and Plan Managed. With a registered Plan Manager on our team, we can provide your plan management as well as a team of Support Workers of your choice to support you in achieving your goals and Daily Living Requirements. Our experienced staff will meet with you and your family and/or carers to discuss your needs and plan and what we can achieve within your budget, ensuring that you are provided with opportunities to participate in the community and have all your personal needs met.

Participate In Community

Participating in the community is a significant contributor to social wellbeing and a crucial plan component for NDIS participants. We work closely with you and your support networks to ensure every activity aligns with your goals and interests, supporting you to connect and take part actively in your community. Our staff will assist you plan, organise and facilitate activities that encourage community engagement and social inclusion.

Palliative Care

Our team understands the importance of providing comfort and dignity during one’s later stages of  life. Our goal is to provide loving, compassionate support and care, working with you and your family to ensure your safety and wellbeing while at home. We work alongside your doctors and nurses to ensure all of your medical and personal needs are met, and can make recommendations for appropriate services as things progress.

Companionship

At A and D In Home Care we understand that Aged and Disability care is more than just providing a service, but also about lending a friendly ear for you to talk to. Through listening to your needs and understanding your personality, we hope to always provide Support Staff that you can ‘gel’ with. Our staff often form lasting bonds with their clients, and look forward to a cup of tea after lunch or a chat on the way to an appointment.

Wound Care/Medical Supervision

Many clients will at times need assistance with their medical care, from helpful reminders about medication to care of wounds or ulcerations. With many of our staff coming from a nursing background, we are well equipped to assist you with your medical needs, working alongside other Healthcare Professionals to ensure wounds and conditions are appropriately treated and tended to.

Light Household Duties

Our Support Workers are more than happy to assist with light household duties in order to maintain a safe, secure and healthy environment in your home. Duties may include meal planning and preparation, clothes washing and ironing, dish washing, light house cleaning and shopping. Our expert staff will work alongside you to keep your house looking fresh – one less thing for you to worry about.

Transportation

Transport is a crucial component of life and allows our clients to maintain independence in attending appointments and running errands. Our Support Workers can arrange safe and convenient transport to appointments, work and community events, while also assisting with shopping or other tasks outside the home. Whether you require regular transport services, or need something arranged infrequently, our Support Workers are here to help.

Personal Care

We understand the loss of independence that sometimes comes with living with a disability or developing into your twilight years. That’s why we provide personal care services to assist you with the personal aspects of life, helping you maintain existing hygiene, grooming and dietary routines. Our personal care services may include assistance with or supervision of showering, bathing, dressing, grooming, continence care and eating, as well as assistance with  mobility when required.

Care Plans

While some clients may have pre-exisiting care plans in place, others are seeking care services for the first time. Our staff can assist you in creating and managing Care Plans that are designed around your needs and goals. When new clients join A & D In Home Care we will meet with you, your family and your carers to discuss your needs and ensure a unique Care Plan is put in place
that is personalised, individualised and flexible, enabling you to achieve your goals and meet your needs.

In Home Support

Our goal is to provide the best quality care to the elderly and people living with a disability, enabling you to live your best life in the comfort of your own home. While our services are consistent with others in the industry, we pride ourselves in a person-centred approach, providing support to you when you most need it. For us, In Home Care is not just about the tasks we do, it’s about helping you feel safe, secure, comfortable and valued in your home.

Support Coordination

If you just received your plan and need help understanding the NDIS and their protocols that can be somewhat difficult at times and at A & D In Home Care we will work with you to ensure you are fully supported in the coordination process of your NDIS Plan.
If you are looking for a support coordination for Mental Health we also have a Psychosocial Recovery Coach that can work alongside you for your complex needs.

Psychosocial Recovery Coach

Recovery coaches offer a more hands-on approach, using their specific knowledge of mental health services, to ensure individuals receive the best support to reach their goals.

Aged Care

At A and D In Home Care, we provide personalised Aged Care services to our local community. Our focus will always be providing the best care possible to our clients, listening to your needs and working with you and your loved ones to present options that best suit you.
